Fill in the blank fields with your specific information and check the applicable Type number(s). Your attorney should review this letter before you send it. When describing what happened, be specific and factual. Include: the date and time; the location; what was said or done; who said or did it (by name and title if known); who witnessed it; and how it affected your child. Use exact words and exact actions. Do not editorialize or draw legal conclusions. State what happened. Send the letter via mail and email. Keep a copy for your records.
